A survey in GoHighLevel is a form split into slides, so people answer a few questions at a time instead of facing one long page. Like a form, it maps answers into contact fields and custom fields, and each completed survey creates or updates a contact. Unlike a form, a survey can use conditional logic to jump to a different slide based on an answer, or to disqualify a respondent and end the survey early.
Surveys are a good fit for qualification and intake: funding applications, quote requests, eligibility checks and booking questionnaires.
In practice
You build surveys under Sites, then Surveys (as of October 2026). Add slides, put fields on each, then set conditional logic on answers, for example "if the answer to Do you own the business? is No, jump to the disqualification slide." Survey settings also control things like the progress bar and what happens after the last slide.
On the automation side, the "Survey Submitted" trigger starts workflows the same way a form does, and you can filter on specific answers or disqualification. A common setup tags qualified and disqualified respondents differently and routes only qualified ones to a rep.
Surveys can include file upload fields too. Files uploaded that way are attached to the contact record, which matters later when you need to download or back them up, because they aren't part of a normal contact export.
Where it shows up in RGS Tools
Backup & Insights's file backup covers files uploaded through survey and form file fields, and rep packages can split contacts into packages with their files, one per rep. Sub-Account Manager's dropdown option manager helps maintain the choice fields surveys depend on.
